Stars - TOP Soccer - Register to play!

We urge you to send in your membership form immediately!  We want to put together teams as soon as possible in order to start playing in August, 2008.

The soccer season runs from the middle of August with games starting in September.  Practices will be one day a week.  Games will be played on Saturdays from September 6th until November 8th, 2008.  Times and locations of games will vary.

Please follow the instructions carefully to register for Stars – TOP Soccer.  Some players may be required to get a Doctors approval before they can begin practice.

PLAYER REGISTRATION

1. COMPLETELY fill out the California Youth Soccer Association, Inc. membership form in blue ink.

2. COMPLETELY fill out the additional player information in blue ink.  Click here for that form. (MS Word Document). 
   
   Please be sure to sign both forms!
   
   
3. All players MUST include a copy of his/her birth certificate or another form of proof of birth.  Do not send in hospital certificates.
4. Please include a 1 by 1 ½ photo of your player.
5. Enclose a check made out to SCCYSL for $50.
