TOPSoccer Stars Club - Fall 2010 Information

Dear TOPSoccer Stars Club Family and Friends:
Welcome to the 2010 Fall Soccer Season!
We had an outstanding first season last year and anticipate that the 2010 season will be even more exciting! We are looking forward to working with your player this soccer season!
- To register for the Fall season – please follow these steps:
- Complete the 1601 CYSA Membership Form.
- Complete the Additional Player Information. (.pdf)
- Consider sponsoring our team – complete the sponsorship form. (.pdf)
- Include payment of $75 – check made out to TOPSoccer Stars
- Mail to:
TOPSoccer Stars (c/o Daryl von Haunalter)

Scotts Valley, CA 95066
- Play dates begin August 28th and end October 30th, 2010. Play dates will be on Saturday. Time and place are to be determined. Download and fill out a Playdate Schedule
- We will provide uniforms for all registered players which will arrive in August.
- Your player will need to wear cleats, shin guards and once the uniforms arrive – the uniform. Please have your player bring a soccer ball and something to drink.
- If you know of any other kids that would enjoy this program – please let them know about it.
- Please be aware that if you leave your player at a TOPSoccer play date unattended by a parent, adult or legal guardian – that TOPSoccer Stars and CYSA will not be held liable for anything that happens to the player while the parent, adult, or legal guardian is not present.
- Please feel free to contact us through our website at www.sccysl.org/stars or by calling us directly (see phone numbers below).
- Contact Daryl if your player is unable to attend a play date as we have to plan for our buddies based on the number of players attending.
